Who Should Complete the Survey
This survey is intended for all Kansas organizations providing care and services to adults age 65 and older, including:

Nursing Facilities and Nursing Facilities for Mental Health
-
Long-Term Care Units of Hospitals
-
Assisted Living and Residential Healthcare Facilities
-
Home Plus and Boarding Care Homes
-
Hospice providers serving clients age 65+
-
Home Health providers serving clients age 65+
-
Programs of All-Inclusive Care for the Elderly (PACE)
Each organization should complete both components of the survey process.
