Director of Health Information
Evergreen Community of Johnson County is seeking a Director of Health Information, who will plan, direct and coordinate activities of the Health Information department and will identify opportunities for improvement. The Health Information department is responsible for quality improvement, release of information, resident admissions, compliance auditor, utilization review, HIPAA compliance, managing and maintaining data bases, physician credentialing and more. The Director of Health Information serves as an advocate for privacy and confidentiality of health information for our residents and will act as the Privacy Officer for the community. The duties of the Director of Health Information are managing and securing resident records. The Director will spend time working with computers and software to ensure that Evergreen is in compliance with federal mandates for electronic storage of resident information. The Director must make sure that these records are accurate and complete. This position must also make sure that databases are secure and may only be accessed by authorized personnel. Responsible for the day to day operations of the Health Information Department. Will interact daily with clinical and administrative staff. Is the bridge between residents’ health information, state and federal government and other regulating agencies. The Director is required to participate in interdisciplinary meetings as necessary and required. Will review medical documentation for accuracy and proper coding. Track, process and evaluate request for release of medical information, requiring knowledge of federal and state laws, as well as HIPAA regulations. Responsible for conducting chart audits, preparing reports and reporting data to administration as required and necessary. QUALIFICATIONS Must meet licensure requirements for the State of Kansas (RHIA or RHIT required). Must be computer literate with strong computer skills on a variety of software programs. PointClickCare experience is preferred, but not required. Must have thorough knowledge of ICD-10-CM coding. Must be organized and detailed oriented. Hospice Chaplain and Bereavement Coordinator
Evergreen Hospice Care is seeking a Hospice Chaplain and Bereavement Coordinator. The Chaplain and Bereavement Coordinator will be a member of the interdisciplinary team who focuses on the spiritual, emotional, and grief support needs of patients, families, caregivers, volunteers, and staff. Completion of a Clinical Pastor Education program or its equivalent. Equivalencies to CPE may include one of the following: evidence of clinical pastoral educational experience that includes supervision of ministry, peer group participation and theological reflection; AAMFT, AAPC, or AMHC certification; D. Min.; or other accredited degree in pastoral care. 3. Master of Divinity from an accredited institution. Food Safety: Assure safe receiving, storage, preparation, and service of food. Protect food in all phases of preparation, holding, service, cooking, and transportation, using HACCP Guidelines. Prepare cleaning schedules and maintain equipment to ensure food safety. Ensure proper sanitation and safety practices of staff. Participate in weekly audits and quality assurance measure. Nutrition Care Plan: Process new diet orders and diet changes; keeps nutrition care plans updates. Complete and assigned MDS section according to required timeline as needed. Determine resident diet needs and develop appropriate dietary plans in cooperation with RD and in compliance with physicians’ orders. Review plan of care related to nutrition status; document concerns that can be resolved, improved or addressed to improve residents nutritional status and eating function. Review, revise, and implement, in cooperation with the IDT, the resident’s nutrition assessment and plan of care. Supports the Registered Dietician as needed. General Knowledge, Skills and Abilities: Skill in motivating, coaching, and supervising foodservice personnel Intermediate computer skills. Mathematical and numerical skills; mechanical aptitude helpful. Has effective written and oral communication skills. Demonstrated organization skills.
