This two-part session will explore the common medication errors in long term care and dive into techniques for prevention. Best practices for medication pass and how you can utilize your consultant pharmacist to help with error prevention will be discussed. The session will conclude with information about how to properly review, manage, learn and move forward from a medication error.
Session Objectives
- Discuss Frequent Medication Errors.
- Discuss Methods of Error Prevention, including in this discussion best practices related to admission and readmission (ex: from brief hospitalization).
- Discuss Medication Pass Observation and Medication Pass Best Practices. How to conduct medication pass observation, and how can the consultant pharmacist help?
- Discuss Medication Error Documentation and Plan of Correction. How do you review a medication error? What are some potential strategies for management and prevention?

The Infection Control Advocate and Resident Education (ICARE) program was developed to address the challenge of applying infection prevention and control measures while ensuring person-centered care in nursing homes. Session participants will learn how to educate, engage, and empower residents to actively participate in quality care improvement with the support of the long-term care ombudsman.
Session Objectives
- Describe the current state, regulations, and infection prevention practices in nursing homes.
- Explain the principles of resident-centered infection prevention and control through the Infection Control Advocate and Resident Education (ICARE) program, resources, and tools.
- Discuss and apply resident-centered infection prevention and control practices through case scenarios and examples, emphasizing benefits and addressing barriers.

- Gain knowledge on known respiratory modalities to adopt and improve Quality of Care in accordance with professional standards of practice.
- Identify what policies and procedures are needed in the facility to eliminate citations and obtain knowledge of infection prevention pathways for all respiratory care.
